• One response per company, ideally completed by a manager responsible for crewing, marine operations, HSEQ or human resources.
• This questionnaire mirrors the 15 dimensions of the Seafarer Questionnaire; seafarers report their day-to-day experience while companies report how policies are put into practice and what resources are provided, so the two sets of answers can be compared.
• For policy or system-related items, select “Yes” if the company has the relevant policy or system in place, “No” if it does not, and “N/A (Don’t know)” if you are unsure. For business-scenario items, select “N/A (Don’t know)” if the company is not involved in the relevant business activity. For items marked “Fill in,” please provide specific figures such as budget, coverage, headcount, or frequency.
• Data will be used solely for academic research on seafarer health and wellbeing and not for any commercial purpose; company and individual information will be kept strictly confidential.
